Reports submitted in this format will be over looked. Any report not in this format will be dismissed.

In all reports, you must include sufficient proof. If the player in question is found to have not committed this offence you will be punished. Although we want to keep the server safe and fun, you need to find limits. Do not post here unless you can prove that this person has committed this offence.
